If your pet is scheduled for a surgical procedure at SF Vet Hospital, completing this Surgical Information Packet is a required step in your pre-op checklist. This packet includes essential instructions and consent, and must be submitted prior to surgery day.

Anesthesia and surgical procedure(s) to be performed:

I, the undersigned owner (or authorized agent of the owner) of the pet identified above, certify that I am eighteen years of age or older and have provided the clinic with a valid photo ID. I authorize the veterinarian(s) at SF Vet Hospital to perform the procedure(s) listed above. I understand that risks are always associated with anesthesia and/or surgery, and I am encouraged to discuss any concerns with the veterinarians or nurses before the procedure(s) begin. My signature on this form indicates that any questions I have regarding the following topics have been answered to my satisfaction:

The reasonable medical and/or surgical treatment options available for my pet.
Sufficient details of the procedure to understand what will be performed.
How fully my pet is expected to recover and the anticipated recovery time.
The most common and the most serious potential complications.
The length and type of follow-up care and any required home restrictions.
The estimated cost of all services.
The expectation of full payment when my pet is discharged.

While I acknowledge that all procedures will be performed to the best of the abilities of the staff at SF Vet Hospital, I understand that no guarantee or warranty has been made regarding the outcome. I agree to pay a deposit toward the estimated fees, assume financial responsibility for the remaining balance, and provide full payment via cash, Visa, MasterCard, Discover, or check at the time my pet is discharged from the hospital.

Should unexpected lifesaving emergency care be required and the hospital staff is unable to reach me, the staff:

Has my permission to provide such treatment. If treatment is provided I agree to pay for such services. Does not have my permission to provide such treatment. If treatment is provided I agree to pay for such services.
